What it feels like
Mid-back pain shows up in a lot of shapes: a deep ache between the shoulder blades, a sharp jab beside the spine when you take a full breath, or a band of tightness that wraps around toward the front of the rib cage. A common pattern we see is pain under the ribs on one side — often the right — that feels worse at night, when you roll over, or after a day hunched over a desk or steering wheel. It can catch you on a deep breath, a cough, or a sneeze.
What's usually behind it
The mid back is where your ribs meet your spine, and each rib forms two small joints at the vertebra. When one of those rib or thoracic joints gets stiff or irritated — from posture, a twist, a heavy lift, or just sleeping wrong — the muscles around it clamp down to protect it. That guarding is what produces the sharp, catch-your-breath quality and the tenderness that radiates around the rib toward the front. It feels alarming, but mechanically it's very treatable.
When to seek care — and when to seek it urgently
Come in if the pain has lasted more than a few days, keeps returning, hurts to breathe, or is disrupting your sleep. But rib-cage and upper-abdominal pain isn't always musculoskeletal. Get evaluated by a physician or urgent care first if your pain comes with chest pressure, shortness of breath, fever, a persistent cough, pain that wraps into the jaw or arm, nausea and vomiting, or pain clearly tied to eating — those can point to the heart, lungs, gallbladder, or other organs rather than the spine. Gentle, specific adjustments restore motion to the stiff thoracic and rib joints, soft-tissue work releases the muscle guarding wrapping around the rib cage, and a short set of breathing and postural exercises keeps the segment moving so the catch doesn't come back. Most patients feel the rib 'unlock' and breathe easier within the first visits.
